Is it possible for proposers to see the evaluation form used by reviewers?

The comments I received from the review of my RFP-IV preliminary proposal include evaluative statements for each of proposal rating areas, such as "showed potential for fundamentally significant results" for the Scientific Merit.  I assume that the statements correspond to different scores or rating categories proposals receive. It would be helpful for proposers to see all of the statements and their corresponding scoring/rating categories for deeper understanding of how their proposals are judged.

The assumption in this question is incorrect. Standard statements which appeared in the pre-proposal feedback documents did not correspond to scores, but were presented to peer reviewers to provide focus for the review criteria in each section and prompt additional comments.
